Seeking a Full Time LCSW Licensed Social Worker to join our team! Spanish speaking is preferred but not required.

We are a Federally Qualified, Non-For-Profit Health Center serving communities in Suffolk County. We are currently seeking a full-time Social Worker to join our team at the Brentwood site.
A Social Worker will be responsible for providing assessments and evaluations for individuals and families, group and individual counseling, advocacy and referral, education and case management for a diverse population.
Essential Duties and Responsibilities:
Interviews patients and family members.
Obtains psycho-social histories of patients.
Provides individual, group, family and material counseling.
Acts as case manager to help patients negotiate the treatment system.
Responsible for keeping patient records current.
Participates in team meetings.
Develops treatment plans with patients as required.
Writes reports and documentation as required.
Consults with other staff regarding treatment of patients.
Coordinate care with other community agencies such as probation, parole, social services, etc.
Reviews case records to monitor quality of care.
Identifies and categorizes each patient’s age-specific grouping of needs, such as those for infant, adolescent or geriatric patients.
Demonstrates knowledge and skill necessary to provide care appropriate to the age of the patients served.
Demonstrates knowledge of the principles of growth and development over the life span.
Minimum Education Requirement:
Master’s Degree in Social Work and LCSW preferred.

Valid Social Work license in the State of New York
Preferred: Bilingual and able to read and write English/Spanish.
