We are seeking an ADVANCED PRACTICE Breast Care Navigator to join our team!

Job Details:
  • The Advanced Practice Navigator incorporates the role of advanced practice providers into clinical practice, education, research and leadership in addressing the health care needs of all customers in the Breast Care Program. This position will serve as the conduit for services to provide a patient with the experience of integrated care. The Advanced Practice Navigator facilitates patients' interactions throughout the care continuum and among service lines including the physician, physician offices, outpatient providers, radiologists, tertiary care centers, other specialties such as surgery and internal departments of the healthcare delivery team. They coordinate care by guiding the patient through the care process and connecting the patient with appropriate services.  Benefits of this position include streamlined care path transitions, assurance of care continuity and improved patient care satisfaction ensuring that patients and families are informed and involved throughout the process.

    Minimum Education


    < >Master's Degree in Nursing or Graduate of an American Medical Association approved Physician Assistant educational program.  


    < >2 years of Nurse Practitioner or Physician Assistant experience  


    < >Registration with the Vermont State Education Department as a Nurse Practitioner and Certified as a Nurse Practitioner by the American Nurses Credentialing Center or equivalent certifying bodiesorRegistration with the Vermont State Board of Medical Practice as a Physician's Assistant and certified as a Physician Assistant by the National Commission on the Certification of Physician Assistants.

    • Knowledge of contractual criteria w/ payers.
    • Effective planning and organizational skills.
    • Evaluation and critical analysis skills.
    • Committed to assessment of own learning deficits and seeks education needed to acquire missing data.
    • Key quality characteristics are positive relationship building; effective written/verbal communication; negotiation; knowledge of contractual criteria with payers; an ability to effect change. 
    • Knowledge of funding sources, healthcare services, human behavior/ dynamics, the healthcare delivery and financing systems, and clinical standards and outcomes.
