Job Summary: We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our healthcare team. The Clinical Pharmacist will be responsible for ensuring the safe and effective use of medications, providing expert advice to healthcare professionals and patients, and contributing to the optimization of patient outcomes through evidence-based practices.
Key Responsibilities:

Medication Management: Review and interpret physician orders to ensure accuracy and appropriateness of therapy. Detect potential medication interactions and contraindications.

Patient Counseling: Provide comprehensive counseling to patients on proper medication usage, potential side effects, and storage. Educate patients on lifestyle modifications and disease prevention strategies.

Clinical Collaboration: Work closely with physicians, nurses, and other healthcare professionals to develop and monitor individualized treatment plans. Participate in multidisciplinary rounds and provide pharmacotherapy recommendations.

Drug Utilization Review: Conduct regular reviews of patient medication regimens to ensure optimal therapeutic outcomes. Monitor patient responses to therapy and make necessary adjustments.

Education and Training: Serve as a resource for healthcare staff by providing education on pharmacological topics, new medications, and best practices. Mentor pharmacy students and residents as applicable.

Regulatory Compliance: Ensure compliance with all federal, state, and local regulations related to pharmacy practice. Maintain accurate and complete documentation of all pharmaceutical activities.
Qualifications:

Education: Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.

Licensure: Current and unrestricted pharmacist license in the state of Vermont, or eligibility to obtain licensure upon hire.

Experience: Completion of an ASHP-accredited residency program or equivalent clinical experience preferred. Prior experience in a clinical setting is advantageous.

Skills: Strong communication and interpersonal skills. Ability to work collaboratively within a multidisciplinary team. Proficiency in electronic health records and pharmacy information systems.
