Job Description

Full-time Social Worker needed at a top facility in New Hampshire!
(Reference Code: KAM)

A well-established hospital-owned Rural Health Clinic in scenic New Hampshire is currently seeking a Master's prepared Licensed Independent Clinical Social Workers (LICSW).

Candidate must work collaboratively with medical providers and other healthcare professionals in a primary care setting by providing emotional support, mental health evaluations, therapy, and case management services to clinic patients. Unique opportunity to lead integrated care development while maintaining a patient panel. Patients are 16 years of age and older.

Must have a current NH license (or be eligible to obtain one), with 2 years of practice experience. 32 or 40 hour per week positions available.
